It is meibomian gland dysfunction — the oil glands along the lid margins stiffen and block, the tear film loses its oil layer, and your tears evaporate faster than you make them. Lubricating drops replace the water. Multiple studies converge on 40°C as the threshold for therapeutic effect, and once daily is recommended deliberately over anything more frequent, because adherence falls as the frequency rises. Normal meibum melts across roughly 19–45°C; in MGD the thickened meibum sits at the top of that range, often above body temperature (37°C) — which is precisely why body heat alone never clears a blocked gland, and why external heat is required.\u003c\/p\u003e\u003cp\u003eHeat it for the time given on the pack, test it against your inner wrist, then rest it across closed eyes for at least ten minutes. To keep one at treatment temperature you would have to reheat it every two minutes for the whole session — which is why almost nobody succeeds with one. Those figures come from a 2024 evidence review in Ophthalmology and Therapy, and they are the whole reason a proper compress exists.
